PERMOHONAN SIJIL AKADEMIK (GANTIAN)

Panduan Permohonan Sijil Akademik (Gantian)

Dokumen-dokumen yang perlu disediakan oleh pemohon adalah seperti berikut:

  1. Laporan kehilangan Sijil Akademik di mana-mana balai polis yang berhampiran.
  2. Surat Akuan Bersumpah dari mana-mana Pejabat Pesuruhjaya Sumpah.
  3. Surat rasmi menyatakan permohonan penggantian Sijil Akademik dan sebab-sebab kehilangan serta menyatakan alamat yang lengkap untuk dokumen tersebut dihantar melalui pos.
  4. Borang Permohonan Sijil Akademik (Gantian). [SILA KLIK DI SINI]
  5. Resit pembayaran.

Kadar bayaran adalah seperti berikut:

    1. Kadar bayaran kehilangan Sijil Akademik adalah RM 200.00.
    2. Kadar bayaran pos adalah sebanyak RM 12.00 untuk Semenanjung Malaysia dan RM 18.00 untuk Sabah & Sarawak.

Kaedah pembayaran adalah melalui laman sesawang https://epayment.uthm.edu.my/ . Sila hubungi pihak Pejabat Bendahari UTHM di talian
07-4537054 sekiranya terdapat sebarang pertanyaan berkaitan proses pembayaran.

Sila hantar dokumen asal pada perkara 1,2,3,4 dan 5 (resit bayaran sebagai bukti bayaran telah dilakukan) kepada alamat seperti di bawah:

Guidelines for Application of Academic Certificate Replacement

The documents required to be submitted by applicants are as follows:

  1. Police report on the loss of the Academic Certificate from any nearby police station.
  2. Statutory Declaration from any Commissioner for Oaths office.
  3. Official letter stating the application for replacement of the Academic Certificate, including reasons for the loss and a complete mailing address for document delivery.
  4. Academic Certificate Replacement Application Form. [PLEASE CLICK HERE]
  5. Payment receipt.

The payment rates are as follows:

  1. The fee for a lost Academic Certificate is RM 200.00.
  2. Postal charges are RM 12.00 for Peninsular Malaysia and RM 18.00 for Sabah & Sarawak.

Payment can be made via the website https://epayment.uthm.edu.my/. For enquiries regarding payment procedures, please contact the Bursary Office at 07-4537054.

Please submit the original copies of items 1, 2, 3, 4, and 5 (payment receipt as proof of payment) to the following address:

Panduan Permohonan Transkrip Akademik (Gantian)

Dokumen-dokumen yang perlu disediakan oleh pemohon adalah seperti berikut:

  1. Laporan polis mengenai kerosakan atau kehilangan Transkrip Akademik di mana-mana balai polis yang berhampiran sekiranya ianya melibatkan kejadian bencana alam sahaja.
  2. Surat Akuan Bersumpah dari mana-mana Pejabat Pesuruhjaya Sumpah sekiranya ianya melibatkan kejadian bencana alam sahaja.
  3. Surat rasmi menyatakan permohonan penggantian Transkrip Akademik dan sebab-sebab kehilangan serta menyatakan alamat yang lengkap sekiranya dokumen tersebut dihantar melalui pos.

    1. Kadar bayaran kehilangan Transkrip Akademik adalah RM 50.00. Bagi kes yang melibatkan bencana alam, hanya bayaran pos sahaja  
        yang perlu dibayar oleh pemohon.
    2. Kadar bayaran pos adalah sebanyak RM 12.00 untuk Semenanjung Malaysia dan RM 18.00 untuk Sabah & Sarawak.

Guidelines for Application of Academic Transcript Replacement

The documents required to be submitted by applicants are as follows:

  1. Police report regarding the damage or loss of the Academic Transcript, obtained from any nearby police station, only if it involves a natural disaster incident.
  2. Statutory Declaration from any Commissioner for Oaths office, only if it involves a natural disaster incident.
  3. Official letter stating the application for replacement of the Academic Transcript, including reasons for the loss and a complete mailing address if the document is to be sent by post.

The payment rates are as follows:

  1. The fee for a lost Academic Transcript is RM 50.00. For cases involving natural disasters, only postal charges are required to be paid by the applicant.
  2. Postal charges are RM 12.00 for Peninsular Malaysia and RM 18.00 for Sabah & Sarawak.
